Download ↓The forms use current Grey Wolf operational details. Uploading the forms and medical certificate online is optional: if you prefer, you can provide them directly at the gym. A valid medical certificate is still required to participate in the activities. Documents uploaded online are stored encrypted outside the public web directory as described in the privacy notice. Photo/video consent is separate and optional. If you do not have an Italian tax code, indicate this in the online process.
